As a part of Yugoslavia, Macedonia had a highly decentralized health care system, run by 30 local municipalities. The system ‘led to significant overprovision and duplication of services.’3 In 1991 new health care legislation established the basis for a health insurance system, together with clarification of the role of providers and the new mode of organisation for the health care system. The Health Insurance Fund is free-standing and directly accountable to parliament.
