Palliative Care Workforce Capacity in Tanzania

Selian Lutheran Hospital

The hospice team has six members: one nurse, two evangelists, one pastor, one social worker and one clinical officer. Kristopher Hartwig is a part time doctor with the programme. The programme is largely volunteer based. The area served is largely Christian. Most of the volunteers are affiliated or linked to the many denominational congregations in the area.

Muheza Hospice Care

PEPFAR funding allowed for a significant increase in staffing in 2004. According to the Muheza Hospice Care Annual Report 2004,24 the present hospice staff comprises:

Medical doctors: two (salaries paid by PEPFAR)

Clinical officers: Two. Bathlomew Bakari is seconded to the hospice from Teule hospital and is studying for the diploma in palliative medicine from Makere University Uganda through Hospice Uganda. Christian Maembe is studying the distance learning diploma in palliative medicine from Oxford Brookes University through Nairobi Hospice (salary paid by RFE/PEPFAR).

Nursing:

  • Christopher Mnzava, district Aids control co-ordinator (seconded from Teule Hospital)
  • Mary Aloyce, palliative care nurse (seconded from Teule Hospital)

PASADA

There are 82 employed members of staff. Twenty four employees work in the dispensaries and the rest are based at the PASADA headquarters. Within the Medical Department there are three doctors, two assistant medical officers, five clinical officers, and 10 nurses. The Orphans’ Department employs six social workers. There are six counsellors in the Counselling Department. Two part-time volunteers assist with computers and the Orphans’ Department. There are 160 volunteers involved mostly in the home care programme. A proposal to Comic Relief (UK) for a partnership project between PASADA and DCHEP to increase the recruitment of volunteers to work in the community was approved in November 2004. This will result in 250 new volunteers being recruited and trained.

Ocean Road Cancer Institute

The palliative care team comprised six employees at the end of 2004:

  • two clinical officers
  • three nurses
  • one social worker

A doctor currently training in palliative care is expected to join the team on completion of studies. Doctors leaving to study and obtaining lucrative employment elsewhere are some of the reasons for being understaffed. Hussein Mtiro explains:

For the last two months things have changed a little because there was a shortage, some doctors left so the team was distributed …And then we are not working now as the team, palliative care team alone, but we are mixed in the departments so we see patients routinely. So we are lacking also some of what we’ve been doing before, we have a weekly training which we are not doing at the moment. Our routine round, three day times a week is, we are not doing it right now, but we see patients just on the ward round as now.25
