Reimbursement & Funding for Services in Tanzania

Selian Lutheran Hospital

Tanzania is an identified country for the USA’s President’s Emergency Programme for AIDS Recovery (PEPFAR). The funding is divided into several tracks for antiretroviral care, and orphan and vulnerable children care. Selian Lutheran Hospital hospice programme is eager to find an effective way to link the support it is receiving from PEPFAR with scaling up and making a difference to a much wider population. Other funding is primarily sourced internationally from churches. As the hospice service is administered by Selian Lutheran Hospital any local fundraising activities focus on funding for the whole institution. Although the hospital charges for its services, these are heavily subsidised from other sources. Hospice patients do not pay for any services. Hospice staff salaries are funded entirely by the hospital

Muheza Hospice Care

Initial funding to establish the five bedded palliative care ward in Teule Hospital came from personal donations from well wishers in UK. The Diana, Princess of Wales Memorial Fund provided money to build the free standing hospice centre that accommodates offices, day centre, counselling and examination rooms and an education centre. In 2004 funding from St Mary’s Church in Oxford, UK was used to add a second consulting room/STI clinic, and a small office to the centre. Securing funds for building extensions is an ongoing problem, although Geneva Global donated US$ 50,000 to pay for an extension of 6 rooms in 2004. The rooms are expected to be in use in 2005. The Elton John Aids Foundation contributed £35,900 in 2004 for running expenses of the hospice when The Diana Princess of Wales Memorial Fund monies were suspended due to internal problems at the end of 2003. This funding also covered the purchase of a vehicle and the implementation of the education programme.

Rapid Funding Envelope (RFE) is a funding initiative by 10 foreign embassies in Tanzania. Administered by Deloitte and Touche, it is dedicated to supporting comprehensive care for HIV positive patients. In November 2003 RFE funded the purchase of a flow cytometer to measure CD4 counts. RFE also provided the hospice with a fluorescent microscope for use in STI diagnosis. Aids Relief Consortium chose Muheza Hospice Care as one of their points of service to receive PEPFAR funding for antiretroviral drugs. An increasing number of patients and hospital staff receive anti retroviral drugs funded by sponsors in the UK. Cytotoxic drugs are funded by individual donors.

PEPFAR funding pays for the salaries of several staff members.

PASADA

Until mid-2003 the main donors of this service were Catholic Relief Services USA and the Diana, Princess of Wales Memorial Fund. Both organisations suffered internal problems during 2003 and had to withdraw their funding commitments leaving PASADA in a serious financial situation. Realising that dependence on a couple of donors was tenuous; the Executive Director, Mary Ash developed links with several other funding agencies. The Stephen Lewis Foundation in Canada took over the home-based care funding for two years. The remaining programmes were divided into mini-projects and applications were made to appropriate donors for each project. Catholic Relief Services have channelled funds from PEPFAR and the Clinton Foundation to support PASADA. With these additional funds, the organisation is financially secure for the immediate future. In early 2003 the organisation had a budget of US$ 350,000. By 2004 this had grown to over US$ 1,000,000.10 The local archdiocese contributes the takings from their Sunday service collections to PASADA. The need to source funding for nutritional support for patients is ongoing.

Ocean Road Cancer Institute

This government hospital is funded entirely by state funds. The palliative care team salaries are paid by the hospital. Free chemotherapy is provided to paediatric patients, primarily for those diagnosed with Burkitts Lymphoma.
